Either cation exchange chromatography is used at a low enough pH that the desired antibody binds to the column while anions flow through, or anion exchange chromatography is used at a high enough pH that the desired antibody flows through the column while anions bind to it. Various proteins can also be separated along with the anions based on their isoelectric point (pI). In proteins, the isoelectric point (pI) is defined as the pH at which a protein has no net charge. When the pH > pI, a protein has a net negative charge, and when the pH < pI, a protein has a net positive charge. Heart failure - Giving 2.5 times of previous oral dose twice daily for those with acute decompensated heart failure is a reasonable strategy. However, daily assessment of clinical response is needed to adjust the subsequent doses. Edema - Volume overload associated with liver cirrhosis, heart failure, or nephrotic syndrome Cerebral edema - intravenous furosemide can be combined with mannitol to initiate rapid diuresis. However, the optimum duration of such treatment remains unknown. Frequent fluid status monitoring is required to prevent intravascular volume depletion which leads to reduced cerebral perfusion. A bolus intravenous dose of 10 or 20 mg of furosemide can be administered and then followed by intravenous bolus of 2 or 3% hypertonic saline to increase the serum sodium level. Pulmonary edema - Slow intravenous bolus dose of 40 to 80 mg furosemide at 4 mg per minute is indicated for patients with fluid overload and pulmonary edema. Such dose can be repeated after 20 minutes. After the bolus, a continuous intravenous infusion can be given at 5 to 10 mg per hour. For those with underlying renal impairment or severe heart failure, up to 160 to 200 mg bolus dose can be given. Hypertension - A systematic review by the Cochrane Hypertension group assessing the anti-hypertensive effects of loop diuretics found only a modest reduction in blood pressure when compared to placebo. ==== Specific populations ==== Age and race do not influence the pharmacokinetics of suvorexant in a clinically meaningfully way. Exposure to suvorexant is slightly higher in women compared to men (Cmax 9% higher, AUC 17% higher), however dose adjustments based on gender are generally unnecessary. Suvorexant exposure is greater in people with higher body mass index, such as obese people (Cmax 17% higher, AUC 31% higher). This is particularly the case in obese women relative to non-obese women (Cmax 25% higher, AUC 46% higher). Suvorexant exposure with a single dose is not greater in people with moderate hepatic insufficiency compared to healthy individuals. However, the half-life of suvorexant at a dose of 20 mg was prolonged from 14.7 hours (range 10–22 hours) to 19.1 hours (range 11–49 hours) in these individuals. Suvorexant exposure is unchanged in people with severe renal impairment and no dosage adjustment is necessary in these individuals.
